Nurses

Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.

Healthcare Assistants

Healthcare assistants support practice nurses with their daily work and carry out tasks such as phlebotomy (drawing blood), blood pressure measurement and new patient checks. They may act as a chaperone when a patient or doctor requests one.

Receptionists

Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

PCN Roles

Health Village and Dearne Valley PCN (Primary Care Network) is a collaboration between three practices – Clifton Medical Centre, Market Surgery and St Ann’s Medical Centre.

There are various roles within the PCN.

Clinical Pharmacist

A PCN Clinical Pharmacist works alongside a multidisciplinary team of health care professionals to support your GP practice in meeting the health and social care needs of patients and helping to improve access to care. They provide support for prescription and medication queries. They undertake clinical medication reviews to support patients. They work to encourage patients to manage common and minor ailments and to promote self-care.

Pharmacy Technician

The Pharmacy Technician work with the PCN Clinical Pharmacist and practice staff to ensure safe, accurate and timely supply of prescribed medication. They also provide support to other clinical staff and reception teams, coordinating discharge summaries and medication reviews, playing a central role in repeat prescribing processes, liaising with our patients, nursing homes, carers, hospital and community pharmacies and responding to medication queries.

Paramedic

A PCN Paramedic is a practice-base role, and they support the GPs to provide on the day emergency services for patients. Their role involves the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions.</

Nurse Associate

The role of the Nursing Associate is to work with the multi professional team and the registered Nurse to assess, plan, organise and deliver clinical care in line with service and patient needs. The nursing associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings.

Care Coordinator

They work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN. They also work closely with other community services and other relevant service providers.

Social Prescriber

The Social Prescriber works alongside a team within the practices and within the Primary Care Network (PCN) and empowers people to take control of their health and well-being. Social prescribing can help to strengthen community resilience and personal resilience whilst reducing health inequalities by addressing the wider determinants of health such as debt, poor housing and physical inactivity by increasing people’s active involvement with their local communities

Physiotherapist

They work as part of a multidisciplinary team across the practices of a Primary Care Network (PCN), with the PCNs GPs and other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.

Digital & Transformation Lead

The Digital & Transformation Lead supports the practices within the PCN to develop and deliver the objectives of the PCN. They are responsible for helping and developing the utilisation of digital tools and implementing transformation work that utilises data or quality improvement tools to improve quality, drive efficiency or improve patient experience.

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
